A Speculative Account of the Effects of Iodine Supplementation at Different Doses

The value of iodine

Most folk in the Thyroid Community understand the requirement for Iodine because the body uses it to synthesize thyroid hormone.

But a variety of cells require iodine for optimum function, as well (e.g. breast, ovaries, pancreas and prostate). So Iodine is important for the body quite apart from supplying the thyroid gland with Iodine. This view is not widely held in the mainstream.

Further, several scientific reports point to widespread iodine deficiency, particularly in the UK and parts of Russia, pointing to the value of Iodine supplementation, at least in some cases.

Yet a problem and a challenge

The capacity of Fluoride and possibly Estrogen Dominance to aggravate dysfunctional Iodine metabolism argue for close attention. Because for those who choose to supplement with Iodine (and there is a strong case for this to prevent breast cancer, plus those diagnosed with Hashimoto’s Disease, see below), the response to such supplementation (based on reported experience of those contributing to Iodine Forums) appears to vary according to:

  1. the baseline Iodine status
  2. the state of the adrenal glands (operating at full strength or in a state of “adrenal insufficiency”?)
  3. whether Companion Nutrients are simultaneously supplemented.
  4. the chosen dose of Iodine
  5. how it is taken (via skin/orally)
  6. the size of the dose
  7. how quickly dosage is introduced

Important Point: For those in the Thyroid Community who have been diagnosed as having Hashimoto’s Disease, Iodine has a reputation for exacerbating Hashimoto’s Disease. However, Iodine may be tolerated provided that the “Companion Nutrients”, referred to above, are in place.

Why are Companion Nutrients important?

Arguably, the adverse effects attributed to Iodine, when used in the situation of Hashimoto’s Disease, are caused by the Iodine-induced, unbalanced and exaggerated activity of the activity of an important and powerful thyroid enzyme: Thyroid Peroxidase, thereby a creating Oxidative Stress and the potential for thyroid cell damage. The Companion Nutrients are designed to support the body’s antioxidant System. Companion Nutrients are used here to prevent damage arising from the unopposed action of Thyroid Peroxidase, as and when Iodine is introduced to the Thyroid Gland, especially in the situation of an inadequate anti-oxidant System.

The purpose of this article

The purpose of this text is to examine the variable experience of those who have reported the effect of supplementing with Iodine and to attempt to explain those experiences according to how the body responds in different metabolic situations. Therefore the purpose is not to influence the reader in any direction (as regards Iodine usage) but to perhaps enable a speculative view of what might happen if/as and when, Iodine is supplemented and why.

I believe that no one should adopt Iodine supplementation without a good deal of reading around the subject, e.g on the STTM and TPAUK websites.

I apologize for the length of this contribution. However, it deals with the complex issue of Iodine effects and it is tricky to give a brief interpretation of this little-understood, but important area, especially for those whose metabolism is out of whack because of the toxic effects of mercury, which has the capacity to damage adrenal, thyroid and methylation function (at least).

So, this text has also been written for those with suspected toxicity (e.g. from dental amalgam) in the hope that the text might explain and emphasize components of the Shade Protocol (designed for the elimination of mercury), which might be ignored by those who, unwisely, are too hasty to get better. 

The right dose of Iodine

I don’t think anyone fully understands what “the right” dose of Iodine is.

If your adrenals are out of whack, Iodine is liable to cause you to crash.

Nor is there agreement on what may be the further results of Iodine supplementation, especially at higher doses.

Some people assert that if you start supplementing with Iodine, two different effects may result, depending on what dose you start at:

Low Dose

Effect: Up-regulation of thyroid synthesis, especially for those who are Iodine deficient (many people are Iodine deficient, so that makes sense). However, if your adrenal function is down, the cells cannot handle the increased levels of stimulation (via thyroid hormone) without a correspondingly elevated level of cortisol.

It is known that thyroid hormone and cortisol work together. Hence, in the situation of adrenal insufficiency (possibly Mercury-induced in the first place, for some) there is the potential for an adrenal failure-induced “Crash” (= feeling dreadful +/- palpitations =”Thyroid toxicity”). So in these two situations… (a] adequate, vs  b]  inadequate adrenal function…taking Iodine may make you feel better or worse

High dose

Effect: Some people report benefit from the introduction of Iodine at a high initial dosage (going against the principle of starting low, increasing slow). This appears not to make sense.

The theory of taking high doses of Iodine

One theory involves acknowledging that Iodine may function as an antioxidant (that Iodine has antioxidant properties, is accepted, although, perhaps not widely so).  The theory goes: Iodine exerts its antioxidant properties but only noticeably at higher doses.

Further, so the theory goes, that antioxidant effect at higher doses tends to overcome the blockage of adrenal function that is creating impaired adrenal function in the first place. Meaning: Low cortisol synthesis and low cortisol levels, because of Mercury blocking cortisol Synthesis by exerting stress on the anti-oxidant system (Iodine usage here, supporting the anti-oxidant system and opposing Mercury).

That too makes a certain amount of sense, because Mercury is known to be able to block cortisol synthesis by acting as an oxidant and exerting stress on the anti-oxidant system.

Speculatively, once your Iodine dosage has (according to this scenario) restored adrenal function by opposing Mercury effects, any further increase may start to be counter-productive (meaning onset of Iodine overdose).

Speculatively, according to dosage therefore, these situations may result sequentially:

1. Baseline: Iodine deficiency in the diet.

Sub-optimal thyroid function with, perhaps, symptoms of Hypothyroidism.

2. Addition of Iodine at low dosage

a) Improves you if adrenal function is adequate
b) Crashes you if you have adrenal insufficiency

3. Addition of Iodine at high dosage

a) Bypasses the “Crash” effects
b) Remits the oxidative stress which is causing the (Mercury-induced) adrenal Insufficiency (with knock-on low Cortisol levels), enables the adrenals to restore Cortisol synthesis:

Result?  Feeling better

4. Supra-Optimal Iodine Dosage

Meaning: even higher dosage than that necessary to restore adrenal function and Cortisol levels:

Speculatively: Iodine Toxicity

Result: You start to feel down again.

Speculative summary of Iodine effects according to dosage

Starting at a “Low” baseline level (Iodine Deficient status) and from there, increasing daily dosage: Feels down-> Take more Iodine-> Feels better->Increase Iodine dosage. More iodine-> Feels worse-> Increase Iodine dosage. More Iodine->Feels better-> Increase Iodine dosage even further: feels worse

This is all theory and is provided for the benefit of those who speculate about what effects Iodine may have.

In preparing the description of this speculative scenario, I am indebted to the contribution (of a lawyer) to an Iodine Forum, who wrote interestingly on this topic as follows:

“Okay. This all-theoretical, mind you, but based on my experience, I think my theory may be correct. At lower doses of iodine, many of the symptoms labeled as bromide detox are identical to adrenal fatigue symptoms: air hunger, low blood sugar, weight gain, headaches, dizziness, fatigue, insomnia, anxiety, palpitations, etc. Another coincidence: the very things that Brownstein etc. recommend for bromide detox are actually things used to alleviate adrenal fatigue: Vitamin C, salt… This may explain many of the symptoms of detox at lower doses. It’s actually adrenal fatigue. Now, iodine can also act as an antioxidant, but only when the body uses excess iodine to make a lipid called delta-iodolactone. But the body will not make this unless it is such a high dose of iodine that the body feels confident that it has enough iodine for its basic needs, so it can use the excess iodine to make this antioxidant lipid. This antioxidant is like 300x more powerful than Vitamin C. I think that you have to take upwards of 100mg of iodine or higher for the body to make this. Because when I take 25mg, I have horrible detox symptoms, but when I take 100mg of iodine I feel normal. I think that at 100mg and up, the body makes delta-iodolactone, and this [is an] antioxidant . It’s the only thing that can account for the fact that people, who can’t take 25mgs without getting sick, can take 150mg without a problem. And there have been about 10 of us who have found this to be true”

The only difference between the lawyer’s interpretation of Iodine effect (at high dosage) and my interpretation:

My interpretation is that at high Iodine dosage, that strong antioxidant effect kicks in, thereby enabling Iodine to resurrect the adrenals (and thus cortisol levels) by opposing the strong oxidant and toxic effects of Mercury. The evidence for this speculation?  At high concentration, iodine appears to tighten up the mitochondrial membrane, thereby preventing leakage of cytochrome C from the mitochondria, across the mitochondrial membrane into the cytosol (cytoplasm).

Result? Less  in the cytosol diminishes oxidative processes in the cytosol.

Result? Less risk of the negative consequences of oxidative stress.

Result? Restoration of the important enzymes which have been suppressed by oxidative stress.

Result? Recovery of 17-Hydroxylase, 5′ Deiodinase, and Methionine Synthase, thereby contributing to the recovery of adrenal, thyroid and Methylation function respectively.

There are those, including myself, who attribute their thyroid & adrenal failure to Mercury toxicity (from dental amalgam). Importantly, this interpretation of Iodine effects draws attention to and emphasizes the importance of supplements described in the Shade Protocol (designed for the elimination of Mercury) and also described as the “Companion Nutrients” in “Iodine Context”.

To summarize: Iodine supplementation is a tricky area. Extensive reading prior to any usage is advised. Adrenal function should be good and the Companion Nutrients should be in place as supplements before embarking. Potential effects of Iodine at different doses have been speculatively described. Getting the right dose is especially tricky and there appears to be no way of knowing what is the right dose without just trying and seeing. Few would argue against a daily dosage of Iodine: one milligram per day, properly supported, as described above.

Further, importantly, this text is not a recommendation to start taking Iodine at massive doses, despite the fact that some report benefit from this approach. The reason for such caution is, as usual, that our metabolisms are all different.

15 Things which Thyroid Patients should teach their Doctors

Screen Shot 2015-08-07 at 4.28.31 PMMany thyroid patients will tell you they have, or have had, doctors they love! I, Janie, have had many of them.

But it doesn’t take away from the fact that those in a medical profession have been sorely lacking for decades about correct knowledge on how to diagnose and treat hypothyroidism or Hashimotos, besides have inappropriate familiarity about all the issues related to being hypothyroid. Even their knowledge on how to correctly read labwork has been lazy.

Because of that poverty of correct knowledge, patients were forced to take the bull by the horns and figure things out for themselves! Stop the Thyroid Madness, the flagship of “patient experiences and wisdom”, represents all that wisdom!

Here are 15 things that any thyroid patient not only has to learn, but needs to teach any medical practitioner the best way they know how:

1) My fatigue and weight gain is not simply because I need to exercise more and eat less.

Granted, we know that exercise and how we eat is important! But being undiagnosed hypothyroid, or poorly treated due to Synthroid or any other T4-only medication, or being held to the TSH, keeps many of us with a low metabolism. The latter results in very easy weight gain, or the failure to do the kind of exercise which would help us!

2) Depression is strongly related to continued hypothyroidism!

We know there can be a variety of reasons for depression, but for most thyroid patients, our depression is a sure sign that we are either undiagnosed due to the lousy TSH lab test, or undertreated due to being on only one of five thyroid hormones like T4-only, or being held hostage to the TSH, a pituitary hormone.

3) The TSH lab test has been a failure for too many years.  

Yes, though a seriously low TSH can detect if we have hypopituitary, for most of us, we’ve had a “normal” TSH yet obvious hypothyroid symptoms. Additionally, when we are optimally treated on Natural Desiccated Thyroid, T4/T3 or T3-only, our TSH lab test is always below range without one hint of bone loss or heart problems. We want to go by the free T3 and free T4, plus symptom removal and a good heartrate and blood pressure instead. //www.stopthethyroidmadness.com/tsh-why-its-useless

4) To figure out if I have Hashimotos,  BOTH antibodies labs need to be tested, not just one. 

To detect if we have the autoimmune version of thyroid problems, patients saw right away that one antibody could be high, but the other one not.  So we need both the anti-peroxidase AND the anti-thyroglobulin lab tests. And by the way, many Hashi’s patients soar on Natural Desiccated Thyroid if they raise it correctly. See #5.

5) Natural Desiccated Thyroid (NDT) has been changing patient lives for years now, just as it did for decades before Synthroid hit the market. 

Though some patients do better on T4-only meds than others…at first..there is simply too many reported experiences by patients for 15+ years that it’s not the way to go. And those same reports show that being on the five hormones that NDT gives makes much more sense.  Even adding synthetic T3 to synthetic T4 has produced better results.

6) I can’t wait six weeks before having a raise!

Thyroid patients found out the hard way that if they stay on a starting dose of NDT (which is usually one grain) longer than a few weeks, the feedback loop causes hypothyroidism to come back with a vengeance in some way or another. 7) My lab results are not about being in the “normal” range.

This was a huge discovery by informed thyroid patients as they kept observing each others lab results for years: it’s about “where” the lab result falls that tells the story…not just because it falls in a suspicious “normal” range based on the testing participants the lab facility chose. //www.stopthethyroidmadness.com/lab-values

8) If I react poorly to NDT, it’s not because NDT isn’t right for me. 

Patients who have had problems with NDT found out that there are five correctible reasons for most of them:  a) being kept on lower doses far too long b) not raising high enough because of being held to the TSH range c) having low iron d) having a cortisol problem 5) having Lyme. 9) Yes, there really is such a thing as adrenal fatigue/adrenal insufficiency/hypocortisolism.

Easily more than 50% of thyroid patients end up with a cortisol problem, either due to being undiagnosed for years thanks to the use of the faulty TSH lab test, or being put on only one of five thyroid hormones–T4. And to learn more about it, one of your doctor’s own colleagues has written a brilliant chapter as to biologically why we get low cortisol, found in the Stop the Thyroid Madness II book, chapter 15. 10) Saliva testing for cortisol is far more accurate than blood testing

Saliva is said to be testing one’s cellular levels of cortisol, plus it does so at four key times during a 24-hour period, which is important to know. And patients found that the results (from reputable companies) fit their symptoms! Whereas blood cortisol testing is measuring both bound and unbound cortisol, and most of the time does NOT fit the symptoms, showing high cortisol when we are really low, or vice versa. //www.stopthethyroidmadness.com/adrenal-info

11) If some or most of my saliva cortisol results are low, there are safe and effective ways to treat it. 

The adrenal area is one which thyroid patients took great time and care to learn, based on what we read from experts, plus our repeated experiences and wisdom. This is where our doctor, need to be open-minded enough to learn from Stop the Thyroid Madness, both on the website and in the revised STTM book, chapters 5 and 6.

12) If I have acid reflux or stomach problems, it’s usually due to low stomach acid caused by our hypothyroid state, not the need for Prilosec (Omeprazole). And some of us need to be off gluten, especially if we have Hashimotos.

i.e. what we need is to restore a better level of acid in our stomachs, which our hypothyroid state lowers–the latter which causes problems in absorbing vitamins and minerals. That’s why we need to put lemon juice or apple cider vinegar in the liquids we use to swallow our meds and supplements. And a large body of us with Hashimotos need to be off gluten.

13) I’m not stupid just because I didn’t go to medical school, plus I live in my own body. So I need you to see us as a team.

14) No, thyroid cancer is not the easy cancer.

15) My thyroid labwork should be done before I take my thyroid meds for the day. 

Patients discovered that the T3 is NDT will peak about two hours after meds are taken, then a slow fall. If patients are on T3-only, it’s a 4-hour peak. We want to measure what still lingers in us, not the peak or rise.

Amazing and brutally honest “Tell It Like It Is” letter to her lousy Endocrinologist

ENDOCRINOLOGIST SOMEECARDFor years, a huge body of thyroid patients have remained hypothyroid, or gotten worse with increasing hypothyroid symptoms, due to believing in their doctors. 

And the worst group of doctors, as reported again and again by thyroid patients, have been Endocrinologists. So when a new person on any patient forum states they are looking for an Endocrinologist, more experienced thyroid patients wince. 

And this is why Stop the Thyroid Madness, and most especially the book, exists–to educate you about successful patient experience and wisdom so that in turn, you can recognize what is good doctoring, and what is NOT…in any physician.

Below is a graphic letter written by an appalled patient about her disappointing experience with her Endocrinologist. Nearly any thyroid patient can identify!

Dear —–,

I am writing to you as an ex-patient.

I saw you as a hypothyroid patient with significant weight gain, extreme fatigue, dry skin and poor concentration. You decided that I was “fine” on 50mcg of Thyroxine because my TSH was 3.9. You also suggested that I attend a bariatric clinic for weight loss.

I saw you a second time with multiple biochemical abnormalities. They were high cortisol, low bicarbonate, high anion gap, detectable CRP, and detectable ANA. You told me that none of these results were anything to worry about. 

I sought another doctor, because whilst you may be comfortable in completely dismissing numerous abnormal results in someone who remains symptomatic of hypothyroidism, I was not comfortable with this.

Since seeking a second opinion, here are the improvements I have experienced:

* no longer gaining weight

* no longer have dry skin

* no longer exhausted 24/7

* bicarbonate is now in the normal range

* anion gap is now in the normal range

* TSH is now 1

* I no longer need to take salt tablets in order to correct salt wasting

I have also received a diagnosis of Adrenal Insufficiency. This was gained via 4 separate saliva samples over a 24-hour period, which allowed me to see my cortisol fluctuations throughout the day. As I explained to you, I felt that my cortisol was too low during the day and too high at night. This is exactly what my salivary cortisol results show.

You ordered a 24-hour urinary cortisol test for me, and I asked if it would show you my fluctuations throughout the day. You said that it would. I want to draw your attention to your completely erroneous statement. I have no doubt that you were simply trying to end the discussion with me.

My urinary cortisol tests were normal, as I expected they would be. But, this normal result did not indicate just how inappropriate my cortisol secretion is. That is, too high at night and too low during the day. I am thankful that I did not remain under your care, as I have no doubt that you would have continued to minimize my concerns regarding my cortisol levels.

The treatment I have received since leaving your care is T3 medication (RT3 and liver enzymes are both abnormal until I correct the cause and then move to Natural Desiccated Thyroid). This is the first time in years that I have seen an improvement in my thyroid health (lowered TSH, increased fT3), and my adrenal health (I no longer require salt tablets). I don’t think you ever bothered to test my aldosterone levels, which I consider to be negligent in a case where the patient is urinating frequently and requiring salt supplementation.

You and your peers are quick to label hypothyroid patients as lazy — not outright, but by your tones and your suggestions. Rather than offer me an increase in Thyroxine, you condemned me to many more months of poor health. You felt I was better off going to a bariatric clinic than changing my medication! And I know that you would never have offered me an alternative thyroid medication. So, under your care, patients such as myself (who have felt no benefit from Thyroxine) are surely just lazy drama queens. Or so your behaviour would indicate. It’s even more distressing that you maintain this attitude despite clinical signs of hypothyroidism (weight gain, dry skin, exhaustion), and despite multiple biochemical abnormalities.

It’s odd that Thyroxine is the standard medication for hypothyroidism, as effective treatment with this requires that T4 is converted to T3. For many patients, it may be a fair assumption that this conversion process will take place. However, for those of us with continued symptoms and biochemical abnormalities despite “adequate” doses, it is clear that something is not happening as it should. For me, the dramatic differences in my health since beginning T3 medication has demonstrated that conversion was not taking place as it should. You and your peers are happy to call me lazy, but I know that a simple medication change is all that was required to regain my health.

I am aware that T3 medication is dangerous for heart health at high doses, and that this fact makes many doctors avoid its use in cases of hypothyroidism. However, I have three reasons to oppose this conduct:

1. Most medications are dangerous in high doses. T3 is not alone in this regard.

2. In someone with hypothyroidism who has experienced no benefit from Thyroxine, supplementing with T3 medication (or even better, natural desiccated thyroid) is just replacing what the body is struggling to create. This does not mean that the patient’s T3 levels are suddenly excessive, or consequently dangerous.

3. The damage done by inadequately treated hypothyroidism is significant, and certainly greater than low doses of T3 supplementation

So, I write to you both despairing at the level of care you provided, and relieved that I’ve found better care elsewhere. I’m sure you’ll ignore this letter, as you and your peers remain adamant that Thyroxine is the only form of treatment, and those who fail to see improvements must themselves be lazy or unmotivated. This attitude is clearly that of people who have never experienced the devastating effects of inadequately treated hypothyroidism. I hope that one day you and your peers will be able to show empathy for patients such as myself, whether you’ve personally experienced hypothyroidism or not. Whilst personal experience of hypothyroidism should not be required in order for doctors to be empathetic, walking a mile in my shoes would no doubt change the way you treat your patients.
